I sent my application to join the Design Team through both of these two forms :

Because i am not sure which is the main application form.
i just want to know if my application is successfully sent or not ? :confused:

Thanks for pointing this out - I have just removed the first page you mentioned so it doesn't distract anyone's attention from the second one - the only form we use right now

I have received it which means that _redrum (the admin in charge of the applications) has received it as well, so there is no need to resend it. Thanks!
